REPORTING FORM

we are committed to accountability and transparency. if you have witnessed, experienced, or heard of any instances of harm within colab spaces and you would like to report it, please do so here.

we consider"harm" to be: emotional, physical, and sexual harassment, discrimination, and any instances/interactions that make you feel unsafe. we will not share your experience with anyone outside the board, unless there is serious concern of you hurting yourself or others. we will help you access any resources you may need. if you wish to report to either the university or law enforcement, the board will help advocate for you and remain available for support. as detailed below, if you choose to leave your name, we will reach out for additional communication. even if you remain anonymous, we will contact those involved.

if your report is about a board member, we have an outside reviewer that will facilitate the process. if you feel more comfortable reaching out to one of the eboard members directly, please email us as listed on the "who we are" page
